| The Ocala Police Department utilizes a new form
called the Citizen Report Form. This form allows
citizens to report misdemeanor crimes with no solid
suspects or leads, and some lost property cases.
Crimes such as stolen bicycles, gas drive offs, and
other crimes that are being reported late can be
submitted on the Citizen Report Form.
Citizens may download and submit this form
for issuance of a case number for your records or
insurance purposes. Upon completion of the form, you may either bring the
form in person to the Records Section, located at 402
South Pine Ave, or mail it to the following address:
