Complaint Forms

The Ocala Police Department uses the forms that you will find below when a citizen has a complaint against an officer or one of our civilian personnel.  Please print out the required form, complete with as much detailed information as possible, sign and return them to either an OPD supervisor, district office or to the front desk at our main office on South Pine Avenue.  You can mail the forms to the Ocala Police Department by using the address you will find below.

We take all complaints seriously and will investigate them fairly.
